Prizes worth up to and including $600 can be claimed by taking your winning ticket to any licensed retailer or regional office in the state, or by visiting the Louisiana Lottery Corporation headquarters. Retailers are required by law to pay out prizes up to and including $50, although it isn’t always guaranteed that they will pay out prizes over this amount so you may have to visit a different store to receive your winnings.
Prizes worth over $600 up to and including $510,000 can be claimed from a regional office or Louisiana State Lottery headquarters. You will need to show a proof of identification and social security number to collect prizes of this amount.
Prizes up to and including $510,000 can also be claimed by mail. You will need to fill out a prize claim form and send your signed winning ticket to the below address. Claiming With a Lottery Pool
If you have won a Powerball jackpot as part of a lottery pool, the winnings can be split between all members of the group. Each individual must fill out a W-9 form at the time of claiming to ensure taxes are paid correctly. One prize check is presented to either one person within the lottery pool, or you can have it paid to a legal entity such as a partnership or a trust.
It is the responsibility of the person receiving the check on behalf of the group to distribute the winnings accordingly. If you form a trust to claim, you must obtain a tax identification number and note this on the partnership agreement or contract, alongside the names and social security numbers of all the members due to receive a share of the money.
Claim Period
Prizes must be claimed within 180 days of a drawing taking place. If you don’t come forward to claim your winnings in this timeframe, the unclaimed money is returned to the prize fund for future drawings.

With the lottery required to give a minimum of 35 percent of their revenue to the state, over $3.5 billion has been raised since 1990.
